Booking Terms & Conditions
Thank you for visiting my website. The following details the terms and conditions of service, ensuring that you are able to make an informed choice regarding the right care for you and your child. Please feel free to contact me for any further information or details that will help put you at ease and aid your decision making.

Information giving
All information within the website does not constitute individual professional advice. I can only provide accurate individualised and professional advice during a clinical consultation.

The consultation process is very important and I will require personal information regarding both yourself and your baby as this may affect the advice I give and therefore the wellbeing of your child may be affected. Please ensure you bring your child’s Red Health Record with you to the appointment also.

Consent
I will require a completed consent form which I will send to you and discuss further in person, to ensure you are able to make the right informed care choice for you and your baby. This will be completed before any assessment or treatment is given and will state that you have read my Privacy Policy.

Information storage

I am fully registered with the Information Commisioners Office and practice in accordance with the Data Protection Act. All records are stored within a Healthcare Software programme Cliniko and are the property of Jas Jones, Registered Midwife, Tongue Tie Practitioner and IBCLC. Please see the Privacy Policy below or ask for further details if required.

Fees
Currently, I do not accept any payments by cheque and at Home Visits I am able to accept cash and card payments. In clinic, both cash and card payments are accepted.

Cancellation

If you wish to cancel your appointment, please contact me directly. If it is with less than 24 hours notice, there is a £45 cancellation fee.

Complaints policy
I hope that you will not be unhappy with my service, however if you do feel that there is a need to make a complaint it will be taken very seriously. Please contact me directly in writing to share your concerns and I will endeavour to resolve your complaint as quickly as possible.
